Anthem is the medical and prescription drug plan insurance carrier.

The PPO plan includes in-network & out-of-network coverage.

Delta Dental manages our dental plan.  Use a Delta Premier or Delta Preferred network dentist to receive higher benefit coverage and no claim filing or balance bill requirements.

EyeMed administers our vision plan via Unum as the insurance carrier.

The plan includes in-network and out-of-network benefits.

The Company matches employee HSA contributions dollar-for-dollar up to $850 Individual or $1,700 Family annually.

Wealthcare administers the Health Savings Account (HSA).
